If you are visiting The Berkshire Medical Practice for the first time, you will be asked to fill in a short registration form before your appointment. This is to help us become familiar with your medical history so that we can give you the best possible medical care.
Please arrive 10-15 minutes before first your appointment to allow sufficient time to complete the form.
